GUELPH – Partners of the Guelph Wellington Ontario Health Team (GW OHT) have announced a partnership with CBI Home Health and ParaMed Home Health to deliver home care through existing primary care teams.
This partnership marks a significant milestone in advancing integrated primary care in Guelph Wellington, officials say, bringing together a comprehensive range of services that allow individuals to receive the care they need from a team they know.
GW OHT is one of seven OHTs selected to deliver innovative, modernized home care models with Ontario Health and Ontario Health @Home.
This is part of the Ontario government’s plan to modernize home and community care in the province.
Integrated patient care teams are dedicated teams of providers who work together closely to meet the needs of patients served by existing primary care teams, officials say.
The Integrated Patient Care Team (IPCT) model brings together primary care, home care, and community providers to meet the needs of people served by six existing primary care teams across Guelph Wellington.
“Our model is based on high performing health systems around the world where dedicated groups of providers work together to meet the needs of a group of patients in an integrated way,” said GW OHT’s director of transformation Emmi Perkins.
“Patients and caregivers told us they need a single team of providers who work together and communicate with each other to help the patient achieve their goals.”
Perkins added, “Providers told us that they don’t always know what care their patients are receiving in the community.
“In this IPCT model, patients will experience home care and primary care providers working together as a single team.”
